Opt Out or Opt In Form

Parents/guardians have the right to withhold student Internet access, prevent the release of student contact or directory information, limit the use of photos of their student, and to opt their student out of specific curriculum or surveys. Complete the applicable sections of this form within seven (7) days of the student’s enrollment in the school or the start of the school year. All students are by default opted into all programs.

You have the option of changing the status of your student at any time, and your choice will remain in effect until you choose to update it by utilizing this same form. Please allow process time for the school to view your status.

Review Curriculum or Instructional Materials

Parents and community members are invited to review any curriculum or core instructional materials in current or proposed use by contacting the school teacher, principal or the CVSD's Learning and Teaching department throughout the year or by reviewing online when available.

Upon a parent or guardian's written request, based on sincerely held religious beliefs, the district will excuse a student from identified lesson(s) or material(s) and provide a reasonable, non-stigmatizing alternative. Excused students will not be penalized academically. Requests should specify the material or unit; and the district will respond within five school days.
