Students wishing to participate in Baseball, Boys or Girls Golf, Boys Soccer, Fast Pitch Softball, Boys or Girls Tennis or Boys or Girls Track must register on Final Forms and be fully cleared in order to participate in any of our spring sports programs.

If you have used Final Forms before parents will need to log into Final Forms and choose the sport under the Spring Sports category. If you have forgotten your password, please do not create a new account. Click on the Get Help box located in the lower right hand corner and request assistance. Once parents have completed any forms, students will receive an email from Final Forms to complete their portion of the registration. Student must have insurance and a current physical that will not expire during the spring sport season.
