SUBMITTAL OF QUALIFICATIONS FOR ARCHITECT & ENGINEERING, MISCELLANEOUS CONSULTANTS AND PROFESSIONAL SERVICES REFERENCE FILE
Firms including but not limited to: licensed architects and engineers, other professionals such as: support services in planning, programming, environmental analysis, soils engineering, hazardous materials consulting, survey consulting, geotechnical consulting, special inspection and testing consulting, traffic engineering consulting, value analysis, constructability review, commissioning services, fire protection, mechanical and electrical engineers, energy studies and other miscellaneous and professional service consultants.
Qualified firms are invited to submit statements of qualifications for entry into the Central Valley School District, Purchasing Department Reference File for consideration on all projects of any size and nature.
Instructions:
Review, Complete, and Submit the information requested below. Be sure to include all services that the Firm is qualified to provide below. Note: Firms with multiple office locations that may enter contracts independently must submit a registration form for each location.
Send a Statement of Qualifications which includes the following basic information that does not exceed 12 pages for identified services:
One copy of the firm’s state business license that indicates the firm’s type of ownership (i.e. domestic profit corporation; public service corporation (P.S.); sole proprietorship; or partnership.)
One copy of any professional license or Certificate of Authorization required by the state which allows the firm to provide the consultant services being offered (i.e. licensed in accordance with RCW 18.08.310, 18.08.370, 18.43.101, and 18.43.070 for applicable firms).
One copy of a Standard Federal 330 Form Part 2 and any other pertinent data to assist in evaluations.
Other supporting information would help us evaluate the firms’ capabilities, experience, and qualifications listed in the advertisement.
Supplemental Information: Provide information to demonstrate your firm's ability to provide each of the Services you identify. At a minimum, list three similar projects and briefly explain the relevance to the type of service. Include the project name, facility size, project cost, completion date, and reference contact information. This supplemental information shall be a maximum of 2 pages of information for each of the Services identified on the next page.

Qualification submittals entered into the reference file may be purged automatically after two years if no revisions, updates, or similar activities have occurred by the consultant on its original submittal contents. It is the responsibility of individual firms to keep file information current.
