Nurses Notes
Immunization Information for Fifth Graders:
All students in Washington State entering sixth grade are required to be immunized against Tdap (tetanus, diphteria and pertussis) and varicella (chicken pox). The Tdap is a new vaccine that is given after a child turns 11 years of age. If your fifth grade child has had chicken pox (the disease) he/she will be considered adequate for sixth grade entry.
A reminder will be sent home at both conferences and at the end of the school year. If your child completes the immunization during this school year, send the information to the school nurse and it will be added to your child's health record. If the immunization is completed during the summer months, be sure to notify the middle school your child will be attending.
