According to the law, an IEP (Individual Education Program) must include the following
His or her present level of educational performance, this may include information concerning: academic achievement, social adaptation, prevocational and vocational skills, sensory and motor skills, self-help skills, and speech and language skills;
Specific special education and related services to be provided and who will provide them; and for how much time per day/week.
Dates for when the services will begin and how long they will continue;
Percentage of the school day in which your child will participate in regular education and special education programs;
Annual goals;
Short-term instructional objectives (individual steps which make up the goals);
Transition plan, when applicable; and
Appropriate objective criteria and evaluation procedures to be used to measure your child's progress toward these goals on at least an annual basis.

Goals and objectives page. It lists the subject area in which your child qualified for Special Education. This page contains your child’s' present levels of performance in the specified subject area, annual goal, short-term objectives, accommodations, and information on any related services.

Continuum of placement. It starts off with the least restrictive environment (general education classroom) and progress to the most restrictive setting (hospital instruction).

Summary of service matrix. This page outlines what services your child will be receiving, the location of services, anticipated frequency, duration of service, and Position(s) Responsible for Designing Monitoring and Evaluating Progress. This page also shows the student’s total amount of time per week in special education and general education. This page also contains program modifications or support for school personnel, and general education supplementary aids and services. If your child qualifies for Extended School Year (Special Education Summer School) this page would contain the goals your child would be working on over the summer.

Information on how the student's progress is going to be evaluated and information on the student's level of participation in state or districtwide assessments. Any accommodations that the student need on statewide and district wide assessments will be listed on this page.

Signature page, which contains the signatures of all the participants in the IEP meeting. If the student is going to be 17 prior to next annual IEP review, the student and his/her parent will sign the transfer of rights section on this page. There is also a dissemination of information statement on this page.
